Upon joining our program, each member receives a comprehensive health assessment. Nurse Care Managers work closely with the member, their family, and caregivers to develop a personalized care plan. This plan addresses the member’s specific medical needs, daily living
assistance, and overall goals for a better quality of life.
Coordination & Support
With a deep understanding of the healthcare system, Care Managers help members navigate medical appointments, coordinate with physicians, and manage medications. They ensure that everyone involved in the member’s care is on the same page, creating a seamless and stress-free experience.
Proactive Problem-Solving
Whether it’s managing a new diagnosis, addressing a health concern, or responding to an emergency, Care
Managers are always ready to step in with expert advice and support. They identify potential issues early, providing solutions that keep members safe, comfortable, and supported at all times.
Caregiver Training & Guidance
Caregivers are an essential part of the Adult Foster Care experience. Nurse Care Managers provide ongoing training, resources, and guidance to help caregivers feel confident and capable. Fromproper medication management to recognizing early signs of health changes, they empower caregivers with the knowledge they need to deliver exceptional care.

Staying on top of a client’s health is key to maintaining stability and comfort. Nurse Care Managers conduct regular visits—at least once a month—to assess the client’s health, track progress, and make any necessary adjustments to the care plan. They also train caregivers
on best practices for daily care and health monitoring.
